Integrated care for chronic diseases in Asia Pacific countries

Overview
Integrated care seeks to improve health-care delivery systems to ensure that patients receive appropriate, equitable and affordable health-care services. Ageing populations, increasing burden of chronic diseases, the need to manage chronic conditions are some factors which have triggered the need for, and implementation of, integrated care models in the Asia Pacific region. This Comparative Country Study presents findings from a scoping review, and case studies on integrated care programmes from six countries in the Asia Pacific region. It presents facilitators and barriers to the design and implementation of integrated care programmes for people with chronic diseases, and outlines policy implications for the development, evaluation and successful implementation of integrated care models for the Asia Pacific region.
